REQUIRED FORMS: The following forms are required to participate in camp. Please have available forms ready to upload at registration. After regisitration, any additional forms will need to be uploaded here. We must have this on file prior to the camps start in order for our staff to ensure your child’s safety at our event.

• Copy of participant's latest physical (within 12 months)
• Copy of participant's medical insurance card (front and back)
